WHS Reporting Portal
Reporting Incidents
All incidents need to be reported (whether major or minor) when an incident, personal injury, near-miss, non-conformance, vehicle accident or property damage occurs. The National WHS Manager must be notified on every occasion. If any of the above are minor, you will need to report them within 24 hours. If they are major, they should be reported immediately, and an incident or injury report must be completed either using the QR code or an Incident Report Form which then must be given to the WHS Manager.
Note: Notifiable incidents e.g. death of a person, serious injury or illness of a person, or a dangerous incident must be reported immediately to WHS Regulator and Management Team.
